Bloodied Insignia is Forever quest 95195. The original objective asks for ten Bloodied Insignias delivered to General Marcus Jonathan in Stormwind City.

Objective count
The original objective marks item 268540 as provided and shows a required count of 10. Follow that total in your active quest log; a provided item does not mean ten additional copies are required. The quest-item page retains its separate tooltip and stack limit.

Reward choice
Choose one of Duty Bound Leggings or Remembrance Armor. The readable record names the choices but does not supply a verified payout or a minimum acceptance level.

Starting context
See the central written pickup table for the older attributed skeleton-drop report. The original quest objective alone does not verify a loot percentage or group sharing behavior.

Evidence and limits
Original Forever quest records supply the objectives and named rewards. The wiki has not tested acceptance, sharing, current completion behavior or payouts. Missing experience or currency fields are not zero values. Minimum acceptance level and exact capture build remain unverified; other WoW editions do not fill these gaps.
