Follow each documented item back to its source. A blank source means the acquisition has not been established; it does not mean the item is unavailable.

Read a source correctly
An NPC mentioned in a quest story is not automatically its quest giver or reward giver. A preview screenshot may establish an item's name or tooltip without establishing how to obtain it. Each item article explains the evidence behind its fields.
When planning an upgrade, check the source, build, level requirement, and any restrictions on the individual page. Unknown levels cannot safely be treated as level 1. These lists do not rank equipment or establish best-in-slot combinations.
